SERVICE NAVIGATOR (Designated Coordinator) - Dakota County

Join us as a Service Navigator at Great Work! We are looking for great communicators who are organized, enjoy relationship-building, and are passionate about our mission of equitable employment for people with disabilities. Service Navigators complete essential administrative responsibilities in support of our team of Employment Specialists including:

  • Preparing for and conducting intake meetings for new referrals

  • Representing Great Work at service planning meetings

  • Overseeing service authorizations

  • Writing, designing, and implementing person-centered plans

  • Regularly collaborating with Great Work colleagues and external stakeholders to deliver the best possible services to our clients

You will office out of your home and will spend the majority of your time working remotely, but will travel throughout Dakota County and Eastern Scott County a few times a week on average for in-person meetings.

The job typically requires about 75% remote work and 25% in-person work attending meetings throughout Dakota and Eastern Scott counties.

Flexible Schedule Options

Service Navigators work full-time and enjoy a flexible schedule. Schedule requirements include:

  • Work between 35 - 40 hours each week

  • Attend mandatory trainings and internal meetings

  • Be available M-F 8:30 am - 4 pm to prioritize work needs as they arise

  • Work 6 hours between the hours of 8:30 am - 4 pm Monday-Thursday

  • Work 2 hours between the hours of 8:30 am - 4 pm on Fridays

Beyond that, Service Navigators can work the rest of their hours when they choose whether that’s standard business hours, early mornings, evenings, late at night, or on weekends.

Benefits

Service Navigators receive the following benefits:

  • Competitive pay ranging from $47,000 - $50,000 at hire depending on qualifications.

  • 8 paid holidays and 9 days of PTO during the first year of employment. 14 days of annual PTO thereafter.

Hybrid Work

Service Navigators work from their home office most of the time but will travel throughout Dakota and Eastern Scott counties on occasion for in-person meetings. On average Service Navigators will have in-person meetings twice each week.

Service Navigators regularly complete remote tasks like:

  • Virtual internal team meetings and virtual client planning meetings

  • Required paperwork and documentation

  • Answering correspondence (email, text, Teams messages, and phone calls)

  • Managing service authorizations

  • Scheduling meetings and appointments and managing your calendar

Service Navigators regularly complete in-person tasks like:

  • Meeting with your clients and other professionals on their support team for planning meetings

  • Occasional in-person internal trainings and Great Work team meetings

Support and Collaboration

Service Navigators do a lot of their work independently but are part of a team of professionals united in a shared passion for more inclusion of people with disabilities in the workforce! We connect and collaborate throughout the week. Service Navigators receive attentive and prompt supervision and support from supervisors and seasoned team members through team training, weekly 1-1 check-ins, and on the fly communications throughout the week.

REQUIREMENTS

Minimum Qualifications (Required)

  • Bachelor’s Degree in human services, social work, psychology, nursing, or a related field.

  • One year (2,080 annual hours) of job-related experience in social services, healthcare, or related field.

  • Must have valid driver’s license, current auto insurance, and vehicle available for business use. *Note that driving clients is not required.

  • Must successfully pass DHS background check

Preferred Qualifications

  • Prior professional experience in disability services strongly preferred, but not required.

  • Prior experience as a Designated Coordinator in Minnesota is strongly preferred, but not required.

  • Prior work experience in employment services for people with disabilities is preferred, but not required.

  • Prior knowledge of and work experience under Minnesota's E1MN framework is preferred, but not required.

Knowledge, Skills, and Abilities needed

  • Ability to manage administrative and meeting responsibilities for a caseload of clients with varying levels of priority

  • Ability to establish and maintain professional and collaborative working relationships with client teams, staff, and all others

  • Excellent written and oral communications skills

  • Ability to ethically document and manage time

  • Computer proficiency using Word and Outlook with proficiency in keyboarding skills.

  • Excellent organizational skills and attention to detail are required

Work Environment:

This position will require a combination of remote work done from your home office and in-person meetings performed throughout Dakota and Eastern Scott counties.
